Kualitas Hidup Pasien Hemodialisis dengan Anemia yang Mendapat Terapi Epoetin Alfa: Studi Cross-Sectional Menggunakan Instrumen EQ-5D-5L

Abstract


Penyakit ginjal kronis (PGK) pada pasien hemodialisis sering disertai anemia yang berdampak pada penurunan kualitas hidup. Penelitian ini bertujuan untuk mengetahui kadar hemoglobin, kualitas hidup dan menganalisis hubungan kadar hemoglobin (Hb) dengan kualitas hidup setelah pemberian epoetin alfa. Penelitian ini merupakan studi observasional analitik dengan desain cross-sectional pada 130 pasien hemodialisis di RS PKU Muhammadiyah Gamping. Pengumpulan data dilakukan selama tiga bulan pemberian epoetin alfa. Data pasien dan pemeriksaan hemoglobin diperoleh dari rekam medis , sedangkan data kualitas hidup diukur menggunakan EQ-5D-5L dan EQ-VAS. Data dianalisis menggunakan analisis Mann–Whitney, Kruskal–Wallis, dan regresi linear berganda (p<0,05). Hasil menunjukkan  bahwa 88,5% belum mencapai target Hb.  Sedangkan nilai rata-rata utility index sebesar 0,48±0,27 dan EQ-VAS 64,50±11,68, menggambarkan kualitas hidup kategori sedang. Tidak terdapat hubungan signifikan antara kadar Hb dengan kualitas hidup, baik pada utility index (p=0,361) maupun EQ-VAS (p=0,758). Analisis multivariat menunjukkan tidak terdapat hubungan signifikan baik dari nilai utilitas maupun dengan skor EQ-VAS. Hasil penelitian ini menunjukkan bahwa kualitas hidup pasien hemodialisis bersifat multidimensional. Belum menunjukkan bahwa kadar Hb berkaitan dengan kualitas hidup. Oleh karena itu, penelitian longitudinal diperlukan untuk mengevaluasi hubungan antara pencapaian Hb dan kualitas hidup secara lebih komprehensif.
