PEMBERDAYAAN KELOMPOK LANSIA DENGAN PROGRAM SIAP KAWAL DIABETES (SIKATES) DENGAN PENDEKATAN KOLABORASI INTERPROFESI MAHASISWA KESEHATAN
Abstract
Abstrak: Upaya edukasi kesehatan mengenai manajemen diri pada lansia dengan Diabetes Mellitus (DM) masih belum berlangsung secara komprehensif dan kolaboratif. Variasi pengetahuan dan keterampilan lansia dalam merawat diri menegaskan perlunya pendekatan yang lebih terstruktur dan berbasis tim. Oleh karena itu, Program SIKATES dengan pendekatan kolaborasi interprofessional dikembangkan untuk meningkatkan kapasitas lansia dalam melakukan skrining dan perawatan diri. Program berlangsung selama enam bulan dan melibatkan tim dosen serta mahasiswa dari keperawatan, fisioterapi, kedokteran, dan gizi. Sebanyak 48 lansia kelompok “Peduli Insani” Mendungan berusia 50–85 tahun berpartisipasi dalam kegiatan ini. Rangkaian kegiatan meliputi skrining DM, penyuluhan kolaboratif mengenai pemeriksaan gula darah, diet DM, aktivitas fisik berupa senam, terapi pengobatan, latihan relaksasi, pendampingan aktivitas bermain angklung sebagai dukungan psikososial, serta evaluasi kadar gula darah. Evaluasi dilaksanakan melalui penilaian proses kegiatan serta pemberian kuesioner sebelum dan sesudah penyuluhan. Hasil kegiatan menunjukkan peningkatan pengetahuan peserta mengenai topik manajemen diri, yaitu pemeriksaan gula darah (50%), pemantauan gula darah (47,9%), pentingnya edukasi kesehatan (37,5%), aktivitas fisik (22,9%), sementara topik terapi farmakologi relatif kecil (2,1%). Kolaborasi interprofesi dalam monitoring glukosa dan pendampingan lansia diabetes efektif meningkatkan pemahaman dan perilaku sehat melalui pelayanan yang terpadu. Temuan ini menegaskan bahwa pendekatan kolaborasi interprofessional dapat menjadi strategi efektif dalam memperkuat kapasitas perawatan diri lansia dengan DM secara holistik dan humanis.
Abstract: Efforts to provide health education on self-management for older adults with Diabetes Mellitus (DM) are still not implemented comprehensively and collaboratively. Variations in the knowledge and skills of the elderly in performing self-care highlight the need for a more structured, systematic, and team-based approach. Therefore, the SIKATES Program, employing an interprofessional collaborative model, was developed to enhance the capacity of older adults in conducting screening and self-care related to DM. The six-month program involved faculty members and students from the fields of nursing, physiotherapy, medicine, and nutrition. A total of 48 older adults aged 50–85 years from the “Peduli Insani” Mendungan group participated in this initiative. The program activities included DM screening, collaborative education on blood glucose examination, DM dietary management, physical activity through exercise, medical therapy, relaxation training, angklung music activities as psychosocial support, and blood glucose evaluation. Program evaluation was conducted through process assessments and pre- and post-education questionnaires. The results indicated increased knowledge among participants on several aspects of self-management, including blood glucose testing (50%), blood glucose monitoring (47.9%), the importance of health education (37.5%), and physical activity (22.9%), while knowledge related to pharmacological therapy demonstrated relatively small improvement (2.1%). Interprofessional collaboration in glucose monitoring and support for older adults with diabetes proved effective in enhancing understanding and healthy behavior through integrated services. These findings emphasize that interprofessional collaborative approaches offer an effective strategy to strengthen self-care capacity among older adults with DM in a holistic and humanistic manner.
